If you agree to enlist our services, we would need the following for you (and your spouse, if married):

•  Social security number

•  Drivers’ license

•  Preferred email address & cell number

•  Names, SSN, DOB, and relationship of any dependents

•  Voided check if you would like to receive direct deposits for any refunds (recommended)

•  All tax documents for the year we’re preparing

•  A list of any questions or concerns you may have for your accountant
